Normally, prospective clients are asked to complete and return a detailed Health and Dietary Questionnaire prior to their initial Full consultation. Because this is in printed form, it will need to be sent to you by post.
However, if you would prefer to save time by downloading the Summary Questionnaire, then please click on the link below. (Please bear in mind, though, that the more information you can provide, the more I may be able to help.)
Once you have completed the Summary Questionnaire, please return it via email to simon@nutricoach.co.uk .
Please ensure that completed Questionnaires are returned at least 48 hours prior to your consultation.
All information provided on the Questionnaire will be treated in the strictest confidence.
